Can anyone help me remember this game PLEASE?

It was either on the NES, SNES or the Sega Genesis. The cover on the box was a woman skeleton/grim reaper wearing a veil or face covering, and was pulling it back from her face. I vividly remember a part where you go to a bathroom and the option pops up to open stall and a woman in a pink dress turns into a skeleton/zombie/grim reaper thing and kills you. Sorry I don't have much more details. I was a kid, and I remember it being really scary, and would love to try and find it again. Thanks in advance!!
